Subject: [PATCH] hpsa: detect controller lockup

From: Stephen M. Cameron <scameron@...rdog.cce.hp.com>

When controller lockup condition is detected,
we should fail all outstanding commands and disable
the controller. This will enable multipath solutions
to recover gracefully.
