Subject: [PATCH -mm] mm: Unify pte_to_pgoff and pgoff_to_pte helpers

Use unified pte_bitop helper to manipulate bits in pte/pgoff bitfield,
and convert pte_to_pgoff/pgoff_to_pte to inlines.
