Hi Laurence,

Please collect blk-mq debugfs log via the following script after the hang is
triggered:

	http://people.redhat.com/minlei/tests/tools/dump-blk-info

This script needn't any parameter.

>From Bart's log, looks the request is requeued by dm-rq, but is never kicked
off, and your log can show if yours is same with Bart's.
