Could you capture a blktrace when this happens?

just:

# mount -t debugfs none /sys/kernel/debug
# blktrace -d /dev/blah -o mytrace &
# do your test
# killall blktrace
# tar cvjf mytrace.tar.bz2 mytrace* 

and attach it.  Might offer clues.
