Subject: [PATCH] afs: Fix fall-through warnings for Clang

In preparation to enable -Wimplicit-fallthrough for Clang, fix multiple
warnings by explicitly adding multiple fallthrough pseudo-keywords
in places where the code is intended to fall through to the next
case.
