Subject: [-mm patch] drivers/pci/quirks.c: cleanup

This patch contains the following cleanups:
- move all EXPORT_SYMBOL's directly below the code they are exporting
- move all DECLARE_PCI_FIXUP_*'s directly below the functions they
  are calling
