Subject: [PATCH] various doc/kernel-parameters fixes

- tell what APIC (by request), MTD, & PARIDE mean
- correct some source file names
- remove IA64 "llsc*=" (seems to have been removed from source tree)
- removel SCSI "53c7xx=" (driver already removed)
