Subject: [2.6 patch] x86/kernel/pci-dma.c cleanups

This patch contains the following cleanups:
- make the following needlessly global code static:
  - forbid_dac
  - iommu_sac_force
  - dma_alloc_pages()
- remove the following unused export:
  - iommu_bio_merge
