Subject: [PATCH] md: make use of list_for_each_entry_safe

From: Cheng Renquan <crquan@...il.com>

The rdev_for_each_list defined in <linux/raid/md_k.h> is really a reinvented
wheel of list_for_each_entry_safe, and replaced in this patch.
