Subject: [PATCH 018/114] x86: remove APIC_INIT / APICFUNC / IPIFUNC

The APIC_INIT() / APICFUNC / IPIFUNC macros were ugly and obfuscated
the true identity of various APIC driver methods.

Now that they are not used anymore, remove them.
