Subject: [GIT PATCH] SCSI bug fixes for 2.6.31-rc5

This is basically a bundle of fixes, the largest swathe of which come
from s390 (zfcp).  The rest are miscellaneous handling and one update to
prevent nasty USB fallout from the extended inquiry changes to support
dif/dix.
