- Remove surplus ghost supplies leftover from older code
  (sleep "regulators").

- Remove "boot_on" from LDO D constraints, we have to
  request, enable and disable this regulator from the
  board code in order for the reference count to reach
  0 and shut off the board at powerdown.

- Add #include <linux/pm.h> to the platform regulator
  file.

- Remove unused supplies and tagged the rails with names
  instead for the boot_on/always_on rails.

- This will require the driver in the regulator tree to
  be applied first and that in turn require some changes
  from the MFD tree so the idea is to postpone the merge
  of this until the merge window post MFD+Regulator merge.
