I think / suspect your major problem is you say above that you use a
512-byte buffer to wipe with.  The kernel is using 4K pages.  So when
you write to a 4K section of the drive for the first time, the kernel
implements read-modify-write logic.

Your i/o failures are almost certainly on the read cycle of the above,
not the write cycle.  You need to move to 4K buffers and you need to
ensure your 4K writes are aligned with how the kernel is working with
the disk.  ie. You need your 4K buffer to perfectly align with the
kernels 4K block handling so you never have a read-modify-write cycle.

Effectively you need your code to do something very similar to:

dd if=/dev/zero of=/dev/sda bs=4K conv=sync,noerror

== more background

1) In general disks do not have media errors on write.  They perform a
blind write that is assumed to work.  Errors do occur on read.  When
that happens the sector is tagged for relocation and on the next write
to that sector an alternate sector from the spares area is used.

If a drive has run out of spare sectors and a write is performed to a
sector tagged for relocation you will get a write error.  But it
should be a fast failure, there is nothing to retry.

2) For testing read i/o error logic, hdparm has the --make-bad-sector
command that will set the above mentioned flag.  It does it by forcing
the crc on the media to be bad.  All reads will therefore fail.

There is a corresponding clear bad sector that restores things to normal.

3) Modern drives to have a way to adjust the read timeout time.  I
don't recall the specifics right now, but since you should only be
writing, it wont help you anyway.
