Subject: [PATCH 2/2] rmap: Fix Bugzilla Bug #5493
Store the vmas associated with an anon_vma in a prio tree instead
of a list.
We must make sure that the prio tree is updated during vma_adjust. This
in turn requires a way to keep an anon_vma around even if its prio tree
is temporarily empty. The ksm_refcount is turned into a general anon_vma
refcount for this purpose.
