Subject: [GIT PATCH] SCSI bug fixes for 2.6.34-rc3

This is the first round of bug fixes ... accidentally got incubated a
bit longer than I expected in -next.  Mainly qla vport issues, but some
notable ones (including the lockdep problem with all the transport
classes).
