There's quite a lot BUG_ON in the code path of sync or umount. If you unplug a usb or sd card before umount or immediately after umount, you will hit problem like this. I am now working on a patch to handle this kind of IO problem more gracefully (at least for sync/umount) by using the error handling API (btrfs_std_error). So far, I am still debugging my patch. Will send it to mailing list for review later. Should be finished in 1 or 2 weeks.
