Subject: [PATCHv6 02/19] usb: otg: Rename usb_otg and usb_xceiv to usb_phy

Renames OTG_STATE_* to USB_PHY_STATE_*,
usb_xceiv_event to usb_phy_event and
otg_state_string() to usb_phy_state_string().
