The backtrace shows sg_scsi_ioctl() being called and that is
only visited when the SCSI_IOCTL_SEND_COMMAND ioctl is used
(IOW not the SG_IO ioctl).

And that ioctl can be used on a bsg node (as well as a SCSI
block node (e.g. /dev/sdb) and a sg node). I would like Tommi
to report if his log showed the output from this line (in
block/scsi_ioctl.c):
      printk(KERN_WARNING "program %s is using a deprecated SCSI ioctl,
             please convert it to SG_IO\n", current->comm);
