Subject: [PATCH 09/31] mips/kvm: Factor trap-and-emulate support into a pluggable implementation.

Many of the arch entry points now dispatch through an ops vector.
This will allow alternate implementations based on the MIPSVZ hardware
support to be added.
There is no substantive code change here, just moving things around.
The biggest change is that the implementation specific data needed by
the vcpu and kvm is held in separate structures, pointed to by
vcpu->arch.impl and kvm->arch.impl, minor changes to the assembly code
was made to traverse these pointers.
