Subject: [PATCHv5 00/11] split page table lock for PMD tables

Alex Thorlton noticed that some massively threaded workloads work poorly,
if THP enabled. This patchset fixes this by introducing split page table
lock for PMD tables. hugetlbfs is not covered yet.

This patchset is based on work by Naoya Horiguchi.

Please review and consider applying.

  - do not use split page table lock on 32-bit machines with
    GENERIC_LOCKBREAK enabled -- otherwise it increases sizeof(struct
    page).
    We probably should fallback to dynamic allocation of page->ptl if
    sizeof(spinlock_t) > sizeof(long);
