Subject: [PATCH v5 19/33] x86, irq: Add for_each_ioapic helper

For hotadd and hotremove ioapic controller, we leave blank
slots in ioapics array.

So we can not use for (i=...) to loop them any more.

Introdue ioapics_mask bitmap to track used ioapics, and use
for_each_ioapic to loop them.
