Subject: [PATCH -mm 1/3] page-cgroup: trivial cleanup

Add forward declarations for struct pglist_data, mem_cgroup.

Remove __init, __meminit from function prototypes and inline functions.

Remove redundant inclusion of bit_spinlock.h.
