Subject: [PATCH 3/3] mm: use VM_BUG_ON() instead of VM_BUG_ON_VMA() and VM_BUG_ON_MM()

Convert VM_BUG_ON_VMA() and VM_BUG_ON_MM() to new VM_BUG_ON(). Dump more
than one data structure where is appropriate.
