Subject: [PATCH v4 3/5] pagemap: rework hugetlb and thp report

This patch moves pmd dissection out of reporting loop: huge pages
are reported as bunch of normal pages with contiguous PFNs.

Add missing "FILE" bit in hugetlb vmas.
