Subject: [PATCH v4 02/10] arch, drivers: don't include <asm/io.h> directly,
 use <linux/io.h> instead

Preparation for uniform definition of ioremap, ioremap_wc, ioremap_wt,
and ioremap_cache, tree-wide.
