Here's what I've currently got.

I can't quite see how to unify the XFS fault & huge_fault handlers yet.
Dave, did you want to do that piece?

DAX now has one dax_fault handler; no __ variations since all callers
were using the __, we can just drop the unused wrappers.

This patch is relative to the patchset I put out before, so there's a lot
of deleting the PUD cases that I had put in.  I'll resplit it for merging.
