Subject: [PATCH 09/14] libnvdimm,
 namespace: expand pmem device naming scheme for multi-pmem

pmem devices are currently named /dev/pmem<region-index>. Preserve the
naming of the 0th device, but add a ".<namespace-index>" for other
devices.
