Subject: [PATCH 20/32] Documentation/md.txt: Convert to ReST markup

- add a title for the document;
- convert some parameters to tables;
- use quote blocks where needed;
- use monotonic fonts for parameters;
- adjust whitespaces and blank lines;
- add it to the user's book.
