Oh I see, the following piece of code should be part of the  
USB_ENDPOINT_XFER_BULK case:

if (usb_endpoint_dir_in(&e->desc)) {
         if (!in)
                 in = e;
} else {
         if (!out)
                 out = e;
}
continue;
