Subject: [PATCH v2] lightnvm: remove hybrid ocssd 1.2 support

Now that rrpc have been removed. Also remove the hybrid 1.2 support
from the core.

Changes since v1:

  * Remove hybrid read/write opcodes.
  * Remove l2p table data structures.
