Subject: [PATCH v3] nvme: trace: add disk name to tracepoints

Add disk name to tracepoints so we can better distinguish between
individual disks in the trace output.

Signed-off-by: Johannes Thumshirn <jthumshirn@...e.de>

---
Changes to v2:
* Pass nvme_ctrl to nvme_complete_rq() and nvme_setup_cmd()
* Fixed typo in commit message (Sagi)
* Dropped Sagi's R-b (it changed quite a lot since v2)
