x86/paravirt: Remove unused paravirt bits

The macros ENABLE_INTERRUPTS_SYSEXIT, GET_CR0_INTO_EAX and
PARAVIRT_ADJUST_EXCEPTION_FRAME are used nowhere.

Remove their definitions.
