Subject: [PATCH v2 10/10] nvme/mdev - generic block IO code

Use the block layer (bio_submit) to pass through the IO to the nvme driver
instead of the direct IO submission hooks.

Currently that code supports only read/write, and it still assumes that
we talk to an nvme driver.
