Subject: [PATCH v2 43/45] KVM: Drop kvm_arch_vcpu_init() and kvm_arch_vcpu_uninit()

Remove kvm_arch_vcpu_init() and kvm_arch_vcpu_uninit() now that all
arch specific implementations are nops.
