How about adding some sanity checks here? For example,

	void *p = ioasid_find(NULL, mm->pasid, NULL);

	if (!p)
		ioasid_set_data(mm->pasid, svm);
	else if (IS_ERR(p) || p != svm)
		return INVALID_IOSASID;
