Subject: [PATCH] iwlwifi: renamed all occurrences of IWL_RATE_COUNT to IWL_RATE_INDEX_COUNT

A FIXME requested to eventaully rename IWL_RATE_COUNT to
IWL_RATE_INDEX_COUNT, as it's a more appropriate name.
