Subject: [PATCH 5/6] pps: embed "dev" in the pps_device

Embed the struct device directly in struct pps_device.
Use device_initialize() + device_add() instead of device_create().

Advantages:
  Avoids a separate allocation.
  Lets us set dev as cdev's parent without the extra get_device() hack.
