Current VLAN macros to CVLAN.
Replace:
MLX4_WQE_CTRL_INS_VLAN
MLX4_CQE_VLAN_PRESENT_MASK
With:
MLX4_WQE_CTRL_INS_CVLAN
MLX4_CQE_CVLAN_PRESENT_MASK
