Subject: [net-next PATCH 08/11] libcxgb,iw_cxgb4,cxgbit: add cxgb_mk_close_con_req()
Add cxgb_mk_close_con_req() to remove duplicate
code to form CPL_CLOSE_CON_REQ hardware command.
