Subject: [PATCH V5 for-next 11/21] RDMA/bnxt_re: Support for AH verbs

This patch implements support for create_ah, destroy_ah, query_ah
and modify_ah verbs.

v3: Removes unwanted macros and does cleanup listed by checkpatch
v5: Adds ib_udata parameter to create_ah verb
