Flip IPv4 to use nhc_exceptions for fnhe_hash_bucket and drop
nh_exceptions from fib_nh.

Simplify the code a bit by converting update_or_create_fnhe,
find_exception and rt_cache_route to take fib_nh_common as the input
over fib_nh.
