Hi
I have been running into this while testing a 3.2 kernel for internal 
release.
I can reliably cause it to happen by running  phoronix-test-suite 
benchmark pts/dbench with 48 clients on xfs or just dbench 48. (I know 
dbench isnt a great test. but I needed a general burn in)

Please CC me if there is anything I can do to help.
