Subject: [PATCH] block: add queue_logical_block_mask() and bdev_logical_block_mask()

Introduce queue_logical_block_mask() and bdev_logical_block_mask()
to simplify code, which replace (queue_logical_block_size(q) - 1)
and (bdev_logical_block_size(bdev) - 1).
