You don't have two cases any more.

I'd swap the code so the short bit is replicated - should be smaller:
if (padc == '-' && cb(state, outstr, w) != 0)
return -1;
while (width > w) {
written += 1;
if (cb(state, " ", 1) != 0)
return -1;
width--;
}
if (padc != '-' && cb(state, outstr, w) != 0)
return -1;
There is also a slight inconsistency between 'width--' and 'written += 1'.
I'd use the postfix operator for both, but that is a matter of taste.
