Subject: [PATCH] dev_alloc_name: fix kernel-doc formatting, typos, etc.

The '%' sign needs to be duplicated so that the kernel-doc tools
will print a '%' sign instead of just printed "ltd".
Also fixes some punctuation/grammar and formatting.
