Looking at code in hinic_free_cmdqs(), isn't this also missing:

	cmdq_type = HINIC_CMDQ_SYNC;
	for (; cmdq_type < HINIC_MAX_CMDQ_TYPES; cmdq_type++)
		free_cmdq(&cmdqs->cmdq[cmdq_type]);
?
